The Registration Process

We continuously strive to assure that our services are delivered in an efficient, caring manner. You can help us expedite your registration and thereby shorten the time you need to be here on the day of your surgery by following these guidelines:

• Please come in (with your child if he or she is the one having surgery) prior to the date of surgery - at least two days before if possible
• Bring a picture ID and your insurance card
• If you are unable to pre-admit, please call us at 352-333-4555 so that one of our staff may provide you with the information you will need about your or your child's surgery and to answer any questions you may have.

The registration process usually takes approximately one hour to complete, but please understand that it may take longer at busy times. Appointments are not required for registration, but you are encouraged to call 352-333-4555 for an appointment. Those without appointments are registered on a first-come, first-served basis.

We encourage you to download, print, and complete one of the following health history forms and bring it with you when you come in to register.

Form for adult patients (ages eighteen and over).

Form for children and adolescents (ages seventeen and younger).
